YOUTH MEDICAL FORMS

Summer Camp 2025

The Urban Farm (TUF) is a licensed Childcare Center located in Denver, Colorado.

Per requirements of the license(s) as issued by the City and County of Denver and the State of Colorado, TUF is mandated to collect specific medical and immunization records for all youth participants of its Summer Camp. Links to download each form may be found below.

Required forms for all youth campers:

*FORM A) RECORD OF PHYSICAL AND MEDICATIONS

*FORM C) CERTIFICATE OF IMMUNIZATION

All completed forms are to be sent to the following email address prior to the first day of camp:

TUF will accept all forms and forms of the same purpose that are dated and signed by BOTH parent/guardian and the medical care provider within one year of the youth’s last day of camp. Forms may also be completed and turned in physically to TUF Staff upon the first day of camp for each youth.

If there are any changes to the medication or application of medications to a youth camper during their camp at TUF, please complete and submit Form B) Medical Change Form to the email address listed above.
